The Key Issues Related to Skilled Clinical
Documentation – 2026 for Operational and Clinical Professionals
Skilled Clinical Documentation is required to justify services in Skilled Facilities to Justify Admission Payment and Regulatory requirements. This session will discuss the risks and liabilities that facilities face when documentation of care is not documented or documentation does not justify payment.
Auditors, insurance companies and surveyors are looking to the Medical Record to describe specific care, observations, interdisciplinary communication and care planning delivery.
This session will review the current issues and documentation habits that do not meet quality requirements to justify and record the delivery of care. Case examples and patterns of quality documentation will be discussed. Suggestions for
education and Clinical experience for new employees will be presented.
A must do session for Administrators, DONs, Clinical and Interdisciplinary staff.
Objective 1: Identify risk issues and payment denial
issues present when Documentation is weak or general.
Objective 2: Review the characteristics of Skilled
Documentation and the complexity of care that should
be included with skilled services.
Objective 3: Discuss case examples of Documentation
entries that meet the skilled requirements.
Objective 4: Identify documentation that will
withstand audit - decrease survey risk – protect
revenue and support Quality reporting.
