Looking Beyond P.D.P.M.

Significant Challenges and Opportunities for Documentation and Payment

The new P.D.P.M. payment system for Part A Medicare claims requires significant coding, documentation and functional scoring from the MDS 3.0 data base. This session will review the changes in coding directions, items impacting payment calculations and the structure of the MDS documents. A must do session for operational and clinical professionals as well as MDS managers. The P.D.P.M. payment system uses a large number of items on the MDS 3.0 that have not impacted payment rates in the past. This new complex payment process requires all IDT members to understand the directions, definitions, and coding instructions in the RAI manual. This session will help all operational and clinical professionals have a better understanding of the documentation required to achieve accuracy and proper payment levels.

1) Identify the data elements that impact the P.D.P.M. rate calculation.

2) Review the changes in the documentation process that must be implemented to assure data accuracy.

3) Describe the responsibilities of the interdisciplinary team to collect and document essential data on the 5 day admission assessment in the first 7 days of the stay.

4) Discuss policy and procedure content to direct the staff documentation in the medical record during the assessment reference period.

5) Identify the data components for the new QRP quality measures related to functional outcomes.

6) Review management approaches and competency activities related to the documentation and data changes.
